Ultra Q: Dark Fantasy is the 19th entry in the Ultra Series, serving as a follow-up to the original Ultra Q. The title card maintains the iconic swirling effect, now accompanied by the additional phrase "Dark Fantasy" beneath it, all presented in black and white.

Ryosei Konishi, Mika Hijii, Ray Fujita, Yasue Sato, Hironobu Kageyama, and Yukijir Hotaru return to portray their characters from the original TV show. Mado Ring Zaruba, the mentor of the original Garo, is a wise figure from the Saezima clan who offers guidance but rarely speaks to Ryuuga. Burai, a Makai Priest partnered with Aguri and mentor to Rian, fights using a Makai Beast named Ragou and shares a mysterious past with Ryuuga. Enhou, the female leader of the special forces task force SG1, suspects Ryuuga's involvement in the appearance of Horrors in Vol City, although she is unsure of his exact role.
